Background and objectivesSepsis is a leading cause of AKI. Animal studies suggest that the pleiotropic effect of statins attenuates the risk for AKI and decreases mortality. This study examined whether statin use was associated with a lower risk for pneumonia-induced AKI and 1-year and cause-specific mortality in patients with AKI.Design, setting, participants, & measurementsMulticenter, prospective cohort study of 1836 patients hospitalized with community-acquired pneumonia.ResultsBaseline characteristics differed among statin users and nonusers. Of the 413 patients (22.5%) who received a statin before hospitalization, statin treatment, when adjusted for differences in age, severity of pneumonia, admission from nursing home, health insurance, and propensity for statin use, did not reduce the risk for AKI (odds ratio [OR], 1.32 [95% confidence interval (CI), 1.02-1.69]; P=0.05). Of patients with AKI (n=631), statin use was associated with a lower risk for death at 1 year (27.8% versus 38.8%; P=0.01), which was not significant when adjusted for differences in age, severity of pneumonia and AKI, use of mechanical ventilation, and propensity score (OR, 0.72 [95% CI, 0.50-1.06]; P=0.09). Among patients with AKI, cardiovascular disease accounted for one third of all deaths.ConclusionsIn a large cohort of patients hospitalized with pneumonia, statins did not reduce the risk for AKI. Among patients with AKI, statin use was not associated with lower risk for death at 1 year. The higher risk for AKI observed among statin users may be due to indication bias.

This paper examined the effect of Hurricane Katrina on children's access to personal healthcare providers and evaluated the use of propensity score methods to compare a nationally representative sample of children, as a proxy for an unexposed group, with a smaller exposed sample. 2007 data from the Gulf Coast Child and Family Health (G-CAFH) Study, a longitudinal cohort of households displaced or greatly impacted by Hurricane Katrina, were matched with 2007 National Survey of Children's Health (NSCH) data using propensity score techniques. Propensity scores were created using poverty level, household educational attainment, and race/ethnicity, with and without the addition of child age and gender. The outcome was defined as having a personal healthcare provider. Additional confounders (household structure, neighborhood safety, health and insurance status) were also examined. All covariates except gender differed significantly between the exposed (G-CAFH) and unexposed (NSCH) samples. Fewer G-CAFH children had a personal healthcare provider (65 %) compared to those from NSCH (90 %). Adjusting for all covariates, the propensity score analysis showed exposed children were 20 % less likely to have a personal healthcare provider compared to unexposed children in the US (OR = 0.80, 95 % CI 0.76, 0.84), whereas the logistic regression analysis estimated a stronger effect (OR = 0.28, 95 % CI 0.21, 0.39). Two years after Hurricane Katrina, children exposed to the storm had significantly lower odds of having a personal health care provider compared to unexposed children. Propensity score matching techniques may be useful for combining separate data samples when no clear unexposed group exists.

He's been sick for about 6ish months now. Not with one long persistent cold or something, just as soon as he gets better from one a little while later he'll get something else. Like he'll have a cold, then as soon as he gets over that he'll get something else like a week later. He can't afford medical insurance at all and none of his illnesses have been life threatening so he won't go to the ER either. It's starting to take a toll on his health I think, he used to be really healthy looking and now he's lost some weight and looks kind of sick all the time. He quit smoking (which he didn't even do that much in the first place) in January and it didn't help anything at all like he thought it would. he washes his hands constantly and eats healthy (when he can eat). i'm starting to get worried about him. He also throws up a lot more than the average person should (not on purpose!) and I know he hates that. I would offer to take him to the doctor but i'm only 14 and he's my legal guardian and i'm living with him as of right now (family issues) so i see him every day and can tell it's getting pretty bad. I asked something like this before and all i got was that he's on drugs or has some sort of std, which he doesn't, so please don't say that hahaha. Any tips on staying well?What could be wrong with him?

So I am 25 right now, and with Obamacare I am covered under my parent's health plan for the remainder of this year. I was wondering if the ruling against the policy, that people are mandated to have health insurance, would affect the policy of children under 26 be covered under their parent's plan. I'm worried because I just was diagnosed with a lifelong disease. Which without insurance would cause me $1400 in pills per month. One more year of not worrying about that would be nice. Thanks, any answers would be appreciative.
