democracynow.org - As the Supreme Court weighs whether the Affordable Care Act goes too far, we host a debate on whether the law goes far enough. The case is reviving the heated tensions that surrounded the health care reform law in the debate leading up to its passage two years ago. Although support for the measure is often equated with backing the expansion of health coverage for all Americans, there are some who maintain it didn't go far enough in helping the uninsured. We speak to Dr. Stephanie Woolhandler co-founder of Physicians for a National Health Program, and John McDonough, who played a key role in shaping Mitt Romney's health care reform law in Massachusetts as well as the Affordable Care Act. The new healthcare law is "going to leave tens of millions of Americans woefully underinsured with gaps in their coverage like co-payments and deductibles, so they will still be bankrupted by illness and it's not going to control cost," Woolhandler argues. "So we still need single payer national health insurance regardless of what happens at the Supreme Court." But McDonough notes that ,"the moment when there would be sufficient political will for the Congress and the president to come together and pass meaningful, near-comprehensive reform — that might take another 20 years. Changing trends in sinus and septal surgery, 2007 to 2009.

BACKGROUND: Sinus surgery remains one of the most commonly performed procedures in the United States, with more than 600,000 patients undergoing sinus operations each year. Advances in our understanding of chronic rhinosinusitis (CRS), as well as the technology used to treat it, has resulted in significant changes in surgical management. The aim of this study was to identify recent changes in the performance and coding of certain endoscopic sinus surgical procedures (ESS) in the hope of generating further discussion and research in this area. METHODS: The MarketScan Research Database was used to query health insurance claims for all rhinological procedures performed between 2007 and 2009. Current Procedural Terminology (CPT) codes for sinonasal procedures that are endorsed by the American Medical Association were analyzed and compared to general otolaryngological procedures in an attempt to identify trends in the performance and coding of certain ESS procedures. RESULTS: Compared to general otolaryngological procedures such as adenotonsillectomy, there was a statistically significant increase in the use of CPT codes for all ESS procedures between 2007 and 2009. The most marked interval changes were seen in the use of the CPT codes for procedures previously considered more complex, namely frontal sinusotomy (75%), sphenoidotomy (33%), and maxillary antrostomy with tissue removal (31%). CONCLUSION: The performance of certain ESS procedures previously considered more complex due to anatomy, access, and proximity to vital structures has increased dramatically in the past few years, whereas increases in basic procedures have mirrored anticipated changes similar to those seen in general otolaryngological procedures.

Open Question: Need advice about ....Breast lift?



I wanted to know has anyone undergone any type of breast surgery? I want to get a breast lift but I'm not sure of the cost....I don't have health insurance but I make pretty good money....I was thinking of paying it out of pocket...I have some money saved. My breast are a 36DD I actually like them but I just want them to sit higher because they fall below my breast line. Also I'm 21 years old ...just looking for some advice...live in Chicago
