Open Question: Bariatric surgery in Michigan with Medicaid?

I'm on disability and am considered morbidly obese. I have sleep apnea, on/off asthma, and am on my way to diabetes and am looking to make a change in my life all around.

I only have Medicaid (no Medicare). Currently, I have the option to change my health care to one of the following (Medicaid only): Blue Cross Complete, Midwest Health Plan, OmniCare Health Plan, or Total Health Care. At the moment I have United Healthcare Community Plan. Of the insurance plans mentioned above, does anyone know which one I would have the least trouble getting the go-ahead for bariatric surgery with?
